The Health Committee is going to have a brown bag pharmacy event on Sunday September 25th, 2016

Between the 1st and 2nd Services  and after the 2nd Service,  We will help everyone fill out a Universal Medical Form

This is a form that lists all you medications that you take (both prescription and over the counter medications)

You keep this form with you and take it to all your medical appointments to provide a convenient list for all your health care providers.

Please gather up all you prescription bottles and over the counter medications and bring them to the Office conference room.

We will give you a printed copy and can e-mail an electronic version to you if you want.
